Case Manager, RN – Compact License

This is a fully remote position, open to applicants in New Jersey.

📋 Description

• Facilitate the provision of relevant benefits and healthcare information to assess eligibility for benefits.

• Encourage participation in wellness initiatives.

• Create, implement, and support health strategies, tactics, policies, and programs.

• Assist in delivering benefits, enhancing overall member wellness, and ensuring timely and effective return to work.

• Conduct assessments, planning, facilitation, care coordination, evaluation, and advocacy.

• Coordinate options and services that address the comprehensive health needs of individuals and families.

• Communicate effectively with members, providers, and community organizations.

• Utilize available resources to foster quality and cost-effective outcomes.

• Collaborate with Community Care teams within a member-focused, team-delivered, community-based care management model.

⛳️ Requirements

• RN Compact license is required.

• At least 5+ years of clinical practical experience with Medicare members; preference for experience in diabetes, CHF, CKD, post-acute care, hospice, palliative care, or cardiac care.

• A minimum of 2+ years in discharge planning and/or home health care coordination experience.

• At least 2+ years of case management experience.

• Strong analytical and problem-solving abilities.

• Excellent communication, organizational, and interpersonal skills.

• Capability to work independently.

• Proficiency in MS Word, Excel, Outlook, PowerPoint, and other proprietary applications.

• Efficient computer skills, including the ability to navigate multiple systems and proficient keyboarding.

• Willingness and ability to obtain multi-state RN licenses if necessary; the company will assist.

• Bilingual candidates are preferred.

• Certification as a Case Manager is preferred.

• Additional national professional certification (such as CRC, CDMS, CRRN, COHN, or CCM) is preferred but not mandatory.

• An Associate degree or Nursing Diploma is required.

• A Bachelor’s degree is preferred.
