Case Manager, Registered Nurse - Field

This is a fully remote position, open to applicants in New Jersey.

📋 Description

• Assess, strategize, execute, and coordinate case management initiatives for members.

• Evaluate the medical needs of members to support overall wellness.

• Create proactive care plans that address identified issues along with both short- and long-term outcomes.

• Utilize clinical tools and data analysis to assess member needs and benefits.

• Apply clinical judgment to mitigate risk factors and tackle complex health and social determinants.

• Conduct assessments leveraging claims data and other information sources, including co-morbidities and multiple diagnoses.

• Employ a holistic approach to determine appropriate referrals to clinical resources and interdisciplinary team members.

• Collaborate with supervisors and healthcare stakeholders to navigate obstacles and achieve objectives.

• Present cases during interdisciplinary case conferences.

• Adhere to regulatory and organizational case management policies and procedures.

• Utilize motivational interviewing techniques to enhance member engagement and evaluate health status and needs.

• Meet members in person within designated areas.

⛳️ Requirements

• A minimum of 3+ years of clinical practice experience.

• Active and unrestricted RN license in the state of New Jersey.

• Willingness and ability to travel 25–50% of the time using personal vehicle to meet members face-to-face.

• Reliable transportation is mandatory.

• Must reside in Ocean County or a nearby county in New Jersey.

• Certification as a Case Manager is preferred.

• A minimum of 2+ years of experience in Care Management, Discharge Planning, and/or Home Health Care Coordination is preferred.

• Ability to work independently and confidently from home while utilizing tools for virtual collaboration.

• Strong analytical and problem-solving abilities.

• Effective communication, organizational, and interpersonal skills.

• Capability to work autonomously.

• Proficient computer skills, including the ability to navigate multiple systems and keyboard proficiently.

• Familiarity with MS Word, Excel, Outlook, PowerPoint, and proprietary applications.

• Bilingual candidates are preferred.

• An Associate’s Degree in Nursing is required.

• A Bachelor's Degree is preferred.
