Case Manager, Registered Nurse – Field

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Collaborate and contribute to the case management process utilizing critical thinking and sound judgment.

• Facilitate optimal healthcare outcomes for members.

• Assist with scheduling appointments.

• Identify and help members access their benefits and educational resources.

• Utilize care management tools and resources effectively.

• Serve dual-eligible Medicare and Medicaid members facing complex health and social challenges.

• Collaborate with members, healthcare providers, and community organizations.

• Address the healthcare and social determinant needs of members.

• Work remotely while being available to travel 50-75% of the time across Will, Kankakee, and surrounding areas.

⛳️ Requirements

• Must be a resident of Illinois.

• Must live in or near Will/Kankakee Counties in Illinois.

• Reliable transportation and the willingness to travel 50-75% of the time from the home location in and around Will, Kankakee, and surrounding areas, IL.

• Minimum of 3-5 years of clinical practical experience.

• Comfort with remote work and being an independent thinker, utilizing tools to collaborate and connect with teams virtually.

• Strong analytical and problem-solving abilities.

• Excellent communication, organizational, and interpersonal skills.

• Capability to work independently.

• Proficient computer skills, including the ability to navigate multiple systems and effective keyboarding.

• Proficiency in standard corporate software applications, including MS Word, Excel, Outlook, and PowerPoint, as well as proprietary applications.

• An Associate's degree is required.

• Must hold an active and unencumbered Registered Nurse license in Illinois.

• Direct/hardwired internet connection to a modem/router within 7 feet of the computer is required.

• Minimum internet speed of 25 Mbps download and 3 Mbps upload is necessary.

• A quiet, secure, and private designated home work location compliant with CVS Health and HIPAA guidelines is essential.

• Must work within the state and city of current residence.

• Familiarity with Microsoft Office 365, Teams, Outlook, Word, Excel, and PowerPoint, or similar Google Workspace experience is preferred.

• Capability to access secure systems such as VPN and EHR portals.

• Ability to manage strong passwords and recognize suspicious emails or links.

• Competence in troubleshooting common technical issues.

• Preferred: 2-3 years of experience in Care Management, discharge planning, and/or home health care coordination.

• Preferred: Certified Case Manager.

• Preferred: Bilingual.
