Case Manager, Registered Nurse

This is a fully remote position, open to applicants in Nevada, +3 more states.

📋 Description

• Provide thorough case management services through both telephonic and in-person interactions.

• Evaluate, strategize, implement, and coordinate care plans to address members' medical needs and enhance overall wellness.

• Execute case management processes and service strategies that align with network management and clinical coverage standards.

• Leverage clinical judgment and data-driven tools to assess member eligibility, recognize health risks, and create proactive short- and long-term care plans.

• Perform comprehensive assessments that take into account co-morbid conditions, functional limitations, and social determinants of health.

• Analyze previous claims, assess work capacity, and identify the necessity for referrals to clinical resources.

• Work collaboratively with supervisors and multidisciplinary teams to navigate obstacles and enhance care planning.

• Grasp member needs and assist in transitioning to suitable programs and services.

⛳️ Requirements

• Current and unrestricted RN license in the state of residence.

• Willingness to acquire additional state licenses upon hiring, with costs covered by the company.

• Minimum of 3 years' experience in acute care and/or case management.

• At least 2 years' experience in Med/Surg.

• Availability to work Monday through Friday, from 8:00 am to 4:30 pm in the local time zone.

• Associate's Degree in Nursing.

• RN License in your state of residence.

• Capability to operate a computer station and sit for prolonged periods.

• Excellent telephonic communication skills.

• Proficiency in managing multiple priorities, demonstrating effective organizational and time management abilities.
