Case Manager – Mental Health Pathways

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Deliver mental health rehabilitation services and support to adults and families.

• Reduce hospitalization and crisis incidents while enhancing community functioning.

• Aid clients in reaching their rehabilitative, resilience, and recovery objectives.

• Ensure continuity of program services in a secure and therapeutic setting.

• Collaborate as a team member in providing mental health services to clients and their families.

• Inspire staff and address challenges related to effective clinical practices.

• Complete necessary documentation within specified timeframes.

• Assume primary case management duties for assigned clients.

• Facilitate individual, group, and family counseling and educational sessions.

• Maintain client service records in accordance with regulations.

• Coordinate communications with admissions, processing, and discharge planning personnel.

• Link services from community resources.

• Apply treatment concepts, evidence-based practices, and service modalities.

• Conduct workshops, lectures, and training for clients, families, staff, and community organizations.

• Represent Rosecrance clients in court appearances as necessary.

• Engage in Clinical Team meetings and related activities.

• Uphold confidentiality in line with the professional Code of Conduct and applicable laws.

• Provide exceptional customer service and positively represent the agency.

• Undertake additional responsibilities as assigned by management.

⛳️ Requirements

• Bachelor's degree in Psychology, Counseling, Social Work, or a related field, or equivalent experience and training.

• Valid driver's license and capacity to meet Agency insurance criteria.

• Must possess personal transportation.

• Self-motivated with the ability to work independently and as part of a team.

• Required computer proficiency.

• Ability to navigate and document within electronic healthcare records is essential.

• Competence in accurately completing required documentation within set timeframes.

• Must successfully pass an occupational health screening.

• Adherence to a zero-tolerance drug-free workplace policy, including marijuana.
