Case Manager – Mental Health Pathways

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Deliver mental health rehabilitation services and support to adults and families.

• Reduce hospitalizations and crisis incidents while enhancing community functioning.

• Assist clients in reaching their rehabilitation, resilience, and recovery objectives.

• Ensure continuity of program services in a secure, therapeutic, ethical, legal, and moral manner.

• Collaborate as a team member in providing mental health services to clients and their families.

• Address recovery issues within clients’ everyday living circumstances.

• Assume primary case management responsibilities for assigned clients.

• Conduct individual, group, and family counseling, as well as educational sessions.

• Maintain client service documentation in compliance with regulations.

• Liaise with admissions, processing, and discharge planning personnel.

• Coordinate services from community resources.

• Apply treatment concepts, evidence-based methods, and service modalities.

• Present workshops, lectures, and training to clients, families, staff, and community groups.

• Attend court on behalf of Rosecrance clients as necessary.

• Engage in Clinical Team meetings and activities.

• Uphold confidentiality in accordance with the professional Code of Conduct and relevant laws.

• Provide outstanding customer service and positively represent the agency.

• Execute other related responsibilities as assigned by management.

⛳️ Requirements

• Bachelor’s degree in Psychology, Counseling, Social Work, or a related field, or equivalent experience and training.

• Possession of a valid driver’s license and ability to fulfill Agency insurance requirements.

• Must have personal transportation.

• Self-motivated individual with the ability to work independently and as part of a team.

• Proficiency in computer use is required.

• Capability to navigate and document in electronic healthcare records is essential.

• Ability to accurately complete necessary documentation within specified time frames.

• Skilled in motivating and resolving issues related to effective clinical practices.

• Ability to adjust verbal and written communication to match the language and style of clients and their families.

• Familiarity with court processes and availability for court appearances on clients’ behalf.

• Must successfully pass an occupational health screening.

• Must operate in a zero-tolerance drug-free environment, including marijuana.
