Case Manager

This is a fully remote position, open to applicants in California.

📋 Description

• Conduct meetings with clients to complete intake processes.

• Assess the unique recovery needs of each client.

• Link clients to suitable services and resources.

• Assist clients in navigating interconnected services and resources.

• Foster relationships with clients and support them throughout their recovery journey.

• Motivate clients to engage with linked resources and opportunities.

• Collaborate with the medical director, nurse practitioners, and counselors to create holistic treatment plans.

• Work autonomously, take responsibility for client care, and build relationships within a cooperative team setting.

• Serve patients throughout California.

• Report directly to the Program Manager for the program.

• Commit to 40 hours per week over 4 to 5 days.

⛳️ Requirements

• Certified or Registered Alcohol or Drug Counselor OR Licensed Therapist (LCSW, ACSW, LMFT, AMFT, etc.).

• A minimum of 1 year of experience in addiction treatment.

• Proficiency in Spanish is a plus.

• Individuals with a criminal record are encouraged to submit their applications.
