This is a fully remote position, open to applicants in Illinois.
• Assess the needs and eligibility of members by utilizing care management tools and reviewing relevant information/data.
• Suggest strategies for case resolution and fulfilling members’ needs by analyzing benefit plans and available programs/services.
• Recognize high-risk factors and service requirements that impact member outcomes and care planning.
• Refer members to clinical case management or crisis intervention services when necessary.
• Organize and execute assigned care plan activities while monitoring progress.
• Collaborate with case managers, supervisors, Medical Directors, and other health programs to address obstacles.
• Present cases during case conferences for multidisciplinary evaluation.
• Identify and escalate issues related to quality of care.
• Negotiate suitable options and services tailored to members’ healthcare needs.
• Utilize influencing techniques and motivational interviewing to engage members and encourage lifestyle or behavioral changes.
• Mentor and support members in making informed medical and healthy lifestyle decisions.
• Assist members in actively participating in healthcare decision-making alongside providers.
• Attend team meetings and engage in peer mentoring/training.
• Implement case management and quality management processes in alignment with regulatory, accreditation, and company standards.
• A minimum of 2 years of experience in Behavioral Health, Social Services, or a relevant field equivalent to program focus.
• Willingness and capability to travel 50-75% of the time across Will, Kankakee, and Grundy Counties and adjacent areas in Illinois.
• Reliable transportation is a must.
• At least 2 years of experience in electronic documentation.
• Proficient in Microsoft Office applications.
• A Bachelor's degree or a non-licensed master's level clinician in Behavioral Health or Human Services such as Psychology, Social Work, Marriage and Family Therapy, or Counseling is required.
• Availability during standard business hours, Monday through Friday, from 8:00 am to 5:00 pm CST.
