This is a fully remote position, open to applicants in Illinois.
• Perform thorough assessments of members' needs and eligibility utilizing care management tools and data analysis.
• Suggest strategies for case resolution and fulfilling member requirements by reviewing benefit plans and available programs/services.
• Recognize high-risk factors and service requirements, referring members to clinical case management or crisis intervention as necessary.
• Coordinate and execute assigned care plan activities while monitoring progress.
• Collaborate with case managers, supervisors, Medical Directors, and other health programs to address obstacles.
• Present cases during multidisciplinary case conferences.
• Detect and escalate quality-of-care concerns.
• Negotiate suitable options and services to address members' benefits and healthcare needs.
• Utilize influencing and motivational interviewing techniques to engage members and encourage lifestyle or behavioral changes.
• Offer coaching, information, and support for independent medical decisions and healthy lifestyle choices.
• Participate in team meetings and provide peer mentoring/training.
• Assist members in actively engaging in healthcare decision-making with providers.
• Implement case management and quality management processes in accordance with regulatory, accreditation, and company standards.
• Travel throughout the Metro East/Southern Illinois area to support members.
• A minimum of 2 years of experience in Behavioral Health, Social Services, or a related field relevant to the program focus.
• Willingness and capability to travel 50-75% of the time within the Metro East, Illinois area.
• Reliable transportation is mandatory.
• At least 2 years of experience with electronic documentation.
• Proficiency in Microsoft Office applications.
• A Bachelor's degree or a non-licensed master's level clinician in Behavioral Health or Human Services (Psychology, Social Work, Marriage and Family Therapy, or Counseling) is required.
• Availability during standard business hours, Monday-Friday, from 8:00 am to 5:00 pm CST.
