This is a fully remote position, open to applicants in Illinois.
• Perform thorough assessments of members’ needs and eligibility utilizing care management tools and data analysis.
• Suggest strategies for case resolution and fulfilling member requirements by assessing benefit plans and available programs/services.
• Recognize high-risk factors and service requirements, ensuring appropriate referrals to clinical case management or crisis intervention.
• Coordinate and execute designated care plan activities while monitoring progress.
• Collaborate with case managers, supervisors, Medical Directors, and other health programs to address challenges.
• Present cases during case conferences for multidisciplinary evaluation.
• Detect and escalate quality-of-care concerns through established processes.
• Negotiate suitable options and services to fulfill members’ benefits and healthcare requirements.
• Apply influencing and motivational interviewing techniques to engage members and encourage lifestyle/behavior changes.
• Offer coaching, information, and support for independent medical and healthy lifestyle decisions.
• Participate in team meetings and provide peer mentoring/training.
• Assist members in making informed decisions with providers regarding their healthcare.
• Implement case management and quality management processes in accordance with regulatory, accreditation, and company standards.
• Minimum of 2 years' experience in Behavioral Health, Social Services, or a related field relevant to the program focus.
• Must be willing and able to travel 50-75% of the time within Winnebago, Boone, Stephenson, Ogle, and Jo Daviess Counties, Illinois.
• Reliable transportation is necessary.
• At least 2 years of experience with electronic documentation.
• Proficiency in Microsoft Office applications.
• A Bachelor's degree or non-licensed master's level clinician in Behavioral Health or Human Services (Psychology, Social Work, Marriage and Family Therapy, Counseling) is required.
• Case management and discharge planning experience is preferred.
• Experience in Managed Care is preferred.
