This is a fully remote position, open to applicants in Illinois.
• Perform thorough assessments of members' needs and eligibility utilizing care management tools and reviewing relevant information/data.
• Propose strategies for case resolution and addressing member needs by examining benefit plans and available programs/services.
• Recognize high-risk factors and service requirements, referring members to clinical case management or crisis intervention when suitable.
• Organize and execute assigned care plan activities while monitoring progress.
• Collaborate with case managers, supervisors, Medical Directors, and other health programs to navigate obstacles to care objectives.
• Present cases during case conferences for multidisciplinary evaluation.
• Identify and report quality-of-care concerns through established procedures.
• Negotiate suitable options and services to fulfill members' benefits and healthcare requirements.
• Employ influencing and motivational interviewing techniques to engage members and encourage lifestyle or behavioral modifications.
• Offer coaching, information, and support for autonomous medical and healthy lifestyle choices.
• Engage in team meetings and provide peer mentoring/training.
• Assist members in making informed healthcare decisions with providers.
• Implement case management and quality management processes in accordance with regulatory, accreditation, and company standards.
• A minimum of 2 years of experience in Behavioral Health, Social Services, or a related field aligned with program focus.
• Willingness and capability to travel 50-75% of the time within Cook County, Illinois, primarily on the west side of Chicago.
• Reliable transportation is mandatory.
• At least 2 years of experience with electronic documentation.
• Proficiency with Microsoft Office applications.
• A Bachelor's degree or a non-licensed master's level clinician in Behavioral Health or Human Services, such as Psychology, Social Work, Marriage and Family Therapy, or Counseling is required.
• Preferred experience in case management and discharge planning.
• Managed Care experience is preferred.
