Case Management Coordinator – Field

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Perform thorough assessments of members’ needs and eligibility utilizing care management tools and reviewing relevant information/data.

• Suggest strategies for case resolution and fulfilling member requirements by analyzing benefit plans and accessible programs/services.

• Detect high-risk factors and service needs, referring to clinical case management or crisis intervention as necessary.

• Coordinate and execute designated care plan activities while monitoring progress.

• Collaborate with case managers, supervisors, Medical Directors, and other healthcare programs to address obstacles.

• Present cases during case conferences for multidisciplinary evaluation.

• Recognize and escalate quality-of-care concerns through established protocols.

• Negotiate suitable options and services to meet members’ healthcare needs.

• Utilize influencing and motivational interviewing techniques to enhance member engagement and promote lifestyle/behavior changes.

• Offer coaching, information, and support for independent medical choices and healthy living.

• Participate in team meetings and provide peer mentoring/training.

• Assist members in making informed decisions regarding their healthcare.

• Implement case management and quality management processes in accordance with regulatory, accreditation, and company standards.

⛳️ Requirements

• Minimum of 2 years experience in Behavioral Health, Social Services, or a related field relevant to the program focus.

• Must be willing and able to travel 50-75% of the time within Cook County, Illinois.

• Reliable transportation is necessary.

• At least 2 years of experience with electronic documentation.

• Proficiency in Microsoft Office applications.

• A Bachelor's degree or a non-licensed master's level clinician is required.

• The degree must be in Behavioral Health or Human Services, such as Psychology, Social Work, Marriage and Family Therapy, or Counseling.
