This is a fully remote position, open to applicants in Illinois.
• Collaborate and support the case management process.
• Facilitate suitable healthcare outcomes for members.
• Assist with scheduling appointments.
• Identify and help members access benefits and education through care management tools and resources.
• Assess members' needs, eligibility, benefit plans, and available programs or services.
• Recognize high-risk factors and service needs, referring to clinical case management or crisis intervention as necessary.
• Coordinate and execute assigned care plan activities while monitoring progress.
• Consult with case managers, supervisors, Medical Directors, and other health programs to address barriers.
• Present cases during case conferences for multidisciplinary review.
• Identify and elevate quality-of-care concerns.
• Negotiate suitable options and services to meet members' benefits and healthcare requirements.
• Employ influencing and motivational interviewing techniques to enhance member engagement and drive behavior changes.
• Provide coaching, information, and support for independent medical and healthy lifestyle choices.
• Participate in team meetings and offer peer mentoring/training.
• Assist members in making informed healthcare decisions.
• Utilize case management and quality management processes in accordance with regulatory, accreditation, and company standards.
• A minimum of 2 years of experience in Behavioral Health, Social Services, or a related field that aligns with program focus.
• Must be prepared and able to travel 50-75% of the time within DuPage, Kane, DeKalb, and Kendall Counties in Illinois.
• Reliable transportation is a must.
• At least 2 years of experience with electronic documentation and proficiency in Microsoft Office applications.
• Preferred experience in case management and discharge planning.
• Managed Care experience is preferred.
• A Bachelor’s degree or a non-licensed master's level clinician qualification is required.
• Degree must be in Behavioral Health or Human Services (such as Psychology, Social Work, Marriage and Family Therapy, Counseling).
