Care Transitions Specialist II

This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Assist individuals as they transition between different care settings by offering care coordination and transition planning services.

• Organize and oversee care transition activities across various settings to facilitate successful discharges and changes in service.

• Create and execute transition plans tailored to individual needs and specific transition requirements.

• Work collaboratively with providers and partners to ensure services are aligned during transition phases.

• Supervise transitions and take action to address any delays or obstacles that may arise.

• Connect individuals with services and guarantee continuity after transitions.


⛳️ Requirements

• Bachelor’s degree in a human services field or licensure as a Registered Nurse (RN) along with three (3) years of relevant experience directly working with individuals experiencing Serious Emotional Disturbance (SED) or Serious Mental Illness (SMI); OR a Master’s degree in a human services field or licensure as a Registered Nurse (RN) with one (1) year of relevant experience working directly with individuals with Serious Emotional Disturbance (SED) or Serious Mental Illness (SMI).

• A valid driver’s license is required.

• Candidates must reside in North Carolina and be within 75 miles of Pitt, Lenoir, or Wayne County.

• Ability to travel within the catchment area as necessary.

• This role does not have licensure or certification prerequisites unless qualifying as a Registered Nurse (RN).

• Preferred: Experience and understanding of peer support, along with experience working with individuals who are certified peer support specialists.

• All applicants must pass a drug test as a condition of employment.
