This is a fully remote position, open to applicants in Washington.
• Engage actively as a member of the transdisciplinary team.
• Serve as the main point of contact for caregivers, families, support networks, and individuals living with chronic conditions.
• Assist caregivers, families, and individuals with chronic conditions to improve quality of life and ensure home safety.
• Conduct biopsychosocial assessments to determine strengths, objectives, unmet health-related social needs, and safety issues.
• Create tailored, culturally sensitive care plans that align with clients’ values and requirements.
• Provide education specific to diseases and stages utilizing the resource library.
• Refer and connect family caregivers and patients to community-based services and support systems.
• Communicate with primary care physicians and healthcare specialists to maintain continuity of care.
• Offer caregiver education that focuses on burnout, self-care, resilience, skills training, and support groups.
• Monitor and maintain contact frequency in accordance with the program model and policies.
• Attend monthly transdisciplinary meetings to collaborate and coordinate care efforts.
• Document all interactions, including presenting issues, objectives, interventions, and action plans.
• Adhere to company policies, procedures, clinical protocols, and confidentiality standards.
• Represent Nolia Health at community events, health fairs, senior centers, and with healthcare partner organizations.
• Inform stakeholders about Nolia Health’s services and foster relationships to generate referrals and community involvement.
• Deliver virtual telehealth care to patients and families in home environments.
• Provide ongoing case management-type support in collaboration with MDs, Neuropsychologists, Nurse Practitioners, and Social Workers.
• Ideally, 2 years of experience in a healthcare or community-based environment.
• Preferably 1 year of professional or personal experience understanding the needs and challenges encountered by family caregivers and older adults.
• Strong understanding of cultural humility.
• Ability to communicate and effectively engage with individuals from various cultures, abilities, genders, ethnicities, and races.
• Experience with Electronic Health Records (EHRs) is an advantage.
• Capacity to manage interactions with caregivers and patients that involve heightened emotions.
• Robust general computer skills, including navigating different systems and resources, troubleshooting minor technology issues, and setting up work equipment.
• Attention to detail along with effective time management and multitasking abilities.
• Familiarity with healthcare, mental health, aging and disability services, or Alzheimer’s disease.
• Proficiency in a language other than English is a plus but not mandatory.
• Access to various training programs, workshops, and resources to enhance skills, knowledge, and expertise.
