This is a fully remote position, open to applicants in Texas.
• Actively engage as a key member of the transdisciplinary team.
• Serve as the main point of contact for caregivers, families, support networks, and individuals living with chronic conditions.
• Assist caregivers, families, and individuals with chronic conditions to improve quality of life and ensure safety at home.
• Perform biopsychosocial assessments to identify strengths, goals, unmet health-related social needs, safety issues, and family dynamics.
• Create tailored, culturally responsive care plans that align with clients' values and requirements.
• Provide education specific to diseases and stages using resources from the library.
• Connect family caregivers and patients with community-based services and support systems.
• Liaise with primary care physicians and healthcare specialists to maintain continuity of care.
• Deliver caregiver education on recognizing burnout, self-care, resilience, skills training, and support groups.
• Monitor and maintain contact frequency in accordance with the program model and policy.
• Participate in monthly transdisciplinary meetings for collaboration and care coordination.
• Accurately document all interactions, including presenting issues, goals, interventions, and action plans.
• Adhere to company policies, procedures, clinical protocols, and client confidentiality standards.
• Represent Nolia Health at community events, health fairs, senior centers, and partner organizations in healthcare.
• Inform stakeholders about Nolia Health services and cultivate relationships that foster referrals and community engagement.
• A minimum of 2 years of experience in a healthcare or community-based environment is preferred.
• Ideally, 1 year of professional or personal experience understanding the needs and challenges faced by family caregivers and older adults.
• A solid grasp of cultural humility is essential.
• Ability to effectively communicate and interact with individuals from various cultures, abilities, genders, ethnicities, and races.
• Experience with Electronic Health Records (EHRs) is advantageous.
• Capable of managing interactions with caregivers and patients that involve heightened emotions.
• Strong general computer skills, including navigating different systems and resources, troubleshooting minor technology issues, and setting up work equipment.
• Attention to detail along with effective time management and multitasking abilities.
• Familiarity with the healthcare industry, mental health, aging and disability services, or Alzheimer’s disease.
• Proficiency in a language other than English is a plus but not mandatory.
• Ability to work with patients and families virtually through telehealth.
• Must be based in Texas.
