Care Navigator, LPN/LVN

This is a fully remote position, open to applicants in Missouri.

📋 Description

• Develop, evaluate, and coordinate care management activities tailored to member needs.

• Create or assist in the formulation of personalized care plans or service plans.

• Inform members and their families or caregivers about available services, benefit options, care alternatives, referrals, and healthcare benefits.

• Assess member needs, identify obstacles to care, and evaluate available resources; recommend and implement suitable plans.

• Identify necessary providers, specialists, and community resources for care.

• Offer psychosocial and resource assistance related to employment, education, housing, nutrition, participant direction, independent living, justice, and foster care.

• Facilitate coordination among members, families, caregivers, and care provider teams to guarantee timely access to services.

• Track progress towards care plan objectives and monitor member status or changes in condition.

• Collaborate with healthcare providers to amend care plans and address member needs.

• Gather, document, and maintain member information and care management activities to ensure regulatory compliance.

• Conduct on-site visits as necessary to evaluate member needs and collaborate with providers/resources.

• Perform additional responsibilities assigned by the team leader and adhere to policies and standards.

⛳️ Requirements

• Bachelor's degree with 2–4 years of relevant experience.

• Alternatively, equivalent experience obtained through applicable knowledge, duties, scope, and skill that reflects the position level.

• Graduation from an Accredited School of Nursing if in possession of clinical licensure.

• Current state clinical license is preferred.

• Availability to work Monday through Friday from 8 am to 5 pm.

• Capability to work remotely from anywhere within the state of Missouri.
