This is a fully remote position, open to applicants in Oregon.

📋 Description

• Develop, evaluate, and coordinate care management activities tailored to member needs.

• Create or contribute to individualized care plans/service plans.

• Provide education to members and their families/caregivers regarding services, benefit options, care alternatives, referrals, healthcare benefits, and provider instructions.

• Assess member needs, identify barriers to care, evaluate available resources, and recommend/facilitate plans to achieve optimal outcomes.

• Identify necessary providers, specialists, and community resources for effective care.

• Offer psychosocial and resource support to facilitate access to local resources and services.

• Coordinate communication between members, families, caregivers, and care provider teams.

• Monitor goals outlined in care plans and track member status or changes in condition.

• Collaborate with healthcare providers to revise care plans and refer members for further evaluation when necessary.

• Collect, document, and maintain member information and care management activities to ensure regulatory compliance.

• Conduct on-site visits when appropriate.

• Perform additional duties as assigned and adhere to all policies and standards.


⛳️ Requirements

• Applicants must reside in Oregon.

• An active, unrestricted Oregon LPN license is required.

• Experience in maternal health is mandatory.

• Background in supporting prenatal, postpartum, or maternal care populations is preferred.

• Experience managing or assisting members with high-risk pregnancies is a significant advantage.

• A Bachelor’s degree is required.

• 2–4 years of relevant experience is necessary.

• Graduation from an Accredited School of Nursing is required if holding clinical licensure.

• Equivalent experience may be considered based on applicable knowledge, duties, scope, and skills.

• Current clinical license from the state is preferred.
