This is a fully remote position, open to applicants in Oregon.
• Develop trusting relationships with patients through effective motivational communication techniques.
• Assess vital signs and utilize the information to encourage positive lifestyle and health behavior modifications.
• Collaborate with patients’ care teams to formulate patient-centered goals and actionable plans.
• Actively track physiological indicators such as blood pressure, weight, and blood glucose levels.
• Deliver education and coaching on preventive health and disease management.
• Conduct monthly wellness calls with assigned patients.
• Facilitate collaborative wellness calls to establish health objectives defined by the Care Team.
• Oversee patient messaging and alerts.
• Guide patients to appropriate physicians for routine inquiries.
• Adhere to escalation protocols for urgent care requirements.
• Engage in synchronous and asynchronous communication with patients through the Remote Care Platform.
• Achieve team and patient engagement program objectives and standards.
• Aid in early interventions to decrease the risks of emergent care and hospital admissions/readmissions.
• Valid unrestricted RN/LVN/LPN license in a compact state and Oregon.
• Availability to work in the PST zone from 8:00 AM to 5:00 PM.
• 1-3 years of clinical experience, particularly in patient management or disease management, is preferred.
• Familiarity with various provider practices and workflows.
• Ability to quickly acquire new knowledge.
• Capacity to work autonomously with minimal supervision.
• Experience with Medicare patients.
• Background in conducting virtual visits with patients and managing care via phone.
• Clinical experience with adult and geriatric patient populations.
• Ideally, experience in phone triage.
• Preferred knowledge of remote-patient monitoring (RPM), Chronic Care Management (CCM), Behavioral Health, Care Coordination, or Utilization Management principles.
• Familiarity with remote working technologies.
• Strong attention to reimbursement requirements.
• Spanish language proficiency is a plus.
• Experience in Motivational Interviewing/Health Behavior Change is advantageous.
• Health Coach certification is a plus.
• Weekends, holidays, and evenings/nights off.
