This is a fully remote position, open to applicants in New York.
• Establish strong relationships with patients through effective motivational communication techniques.
• Assess vital signs and utilize data to encourage positive lifestyle and health behavior modifications.
• Partner with patients’ care teams to set patient-centered goals and develop action plans.
• Actively monitor remotely submitted blood pressure, weight, and blood glucose data.
• Deliver preventive health education and disease management coaching.
• Conduct monthly wellness calls with assigned patients.
• Facilitate collaborative wellness discussions to outline health objectives defined by the Care Team.
• Oversee patient messaging and alerts.
• Guide patients to appropriate treating physicians for routine inquiries.
• Adhere to escalation pathways for urgent care requirements.
• Engage in synchronous and asynchronous patient communication via Vivo Care’s Remote Care Platform.
• Achieve team and patient engagement program objectives and reimbursement standards.
• Valid RN/LPN/LVN license in a compact state and New York without restrictions.
• Availability to work in the EST zone from 8:00 AM to 5:00 PM.
• 1-3 years of experience in clinical settings, patient management, or disease management is preferred.
• Familiarity with various provider practices and workflows.
• Quick learning ability.
• Capacity to work independently with minimal supervision.
• Experience with Medicare patients.
• Background in conducting virtual visits with patients and managing telephonic care.
• Clinical experience with adult and geriatric populations.
• Proficiency in Spanish is a plus.
• Experience in Motivational Interviewing/Health Behavior Change is a plus.
• Health Coach certification is an asset.
