This is a fully remote position, open to applicants in Massachusetts.
• Establish trustworthy relationships with patients through motivational communication strategies.
• Assess physiological indicators and utilize the information to encourage positive lifestyle and health behavior modifications.
• Partner with patients’ care teams to conduct wellness outreach calls.
• Set patient-focused objectives and create related action plans.
• Appropriately monitor blood pressure, weight, and blood glucose levels.
• Achieve team targets, benchmarks, metrics, and goals for patient engagement programs.
• Promote ongoing patient involvement in remote monitoring initiatives.
• Deliver educational resources and coaching on preventive health and disease management.
• Conduct monthly wellness calls with designated patients.
• Facilitate collaborative wellness calls to establish health objectives set by the care team.
• Oversee patient messaging and notifications.
• Refer patients to their physicians for routine inquiries.
• Adhere to appropriate escalation protocols for urgent care requirements.
• Engage in both synchronous and asynchronous communication with patients through Vivo Care’s Remote Care Platform.
• Actively monitor vital signs to mitigate the risk of emergency care and hospital admissions/readmissions.
• Valid RN/LPN/LVN license without restrictions in a compact state and Massachusetts.
• Availability to work in the EST time zone between 8:00 AM and 5:00 PM.
• 1–3 years of clinical experience, patient management, or disease management preferred.
• Familiarity with various provider practices and workflows.
• Capability to work independently with minimal supervision.
• Experience with Medicare patients is valuable.
• Proficiency in conducting virtual visits and telephonic care management.
• Clinical experience with adult and geriatric populations.
• Ideally, experience with phone triage.
• Preferred knowledge of remote-patient monitoring (RPM) and/or Chronic Care Management (CCM), Behavioral Health, Care Coordination, or Utilization Management principles.
• Familiarity with remote work technologies.
• Experience in Motivational Interviewing/Health Behavior Change is a plus.
• Health Coach certification is advantageous.
• Proficiency in Spanish is a bonus.
• Interest in pursuing professional leadership development and growth opportunities.
