This is a fully remote position, open to applicants in Illinois.
β’ Establish trusting relationships with patients through effective motivational communication techniques.
β’ Assess vital signs and utilize data to encourage positive lifestyle and health behavior modifications.
β’ Work in partnership with patients' care teams to conduct wellness calls and set patient-focused objectives.
β’ Create action plans aimed at enhancing patients' health and overall well-being.
β’ Monitor blood pressure, weight, and blood glucose levels with clinical appropriateness.
β’ Achieve team targets, standards, metrics, and goals related to patient engagement programs.
β’ Foster ongoing patient involvement in remote monitoring initiatives.
β’ Deliver education and coaching on preventive health and disease management.
β’ Conduct monthly wellness calls with designated patients.
β’ Oversee patient messaging and alerts.
β’ Refer patients to treating physicians for routine inquiries.
β’ Adhere to escalation protocols for urgent care requirements.
β’ Facilitate synchronous and asynchronous patient communication via Vivo Care's Remote Care Platform.
β’ Actively monitor electronically transmitted physiological indicators to minimize the risk of emergency care and hospital admission/readmission.
β’ Unrestricted RN/LPN/LVN license in a compact state; Illinois is preferred.
β’ Availability to work in the CST time zone from 8:00 am to 5:00 pm.
β’ 1β3 years of experience in clinical settings, patient management, or disease management is desirable.
β’ Familiarity with various provider practices and workflows.
β’ Ability to work autonomously with little supervision.
β’ Experience working with Medicare patients.
β’ Proficiency in conducting virtual visits and telephonic care management.
β’ Clinical experience with adult and geriatric patient demographics.
β’ Knowledge of remote working technologies.
β’ Spanish language skills are a plus.
β’ Experience in Motivational Interviewing/Health Behavior Change is advantageous.
β’ Health Coach certification is a plus.
β’ Weekends, holidays, and evenings/nights off.
