Care Navigator – Foster Care

This is a fully remote position, open to applicants in Missouri.

📋 Description

• Design, evaluate, and oversee care management initiatives tailored to the needs of members.

• Create or contribute to individualized care plans/service plans.

• Inform members and their families/caregivers about available services, benefit options, procedures, care alternatives, referrals, and healthcare benefits.

• Assess member needs, obstacles to care, and available resources, while facilitating plans for optimal outcomes.

• Identify necessary providers, specialists, and community resources for care.

• Offer psychosocial and resource support to members, caregivers, and care managers.

• Coordinate communication between members, families, caregivers, and care provider teams to ensure timely access to services.

• Track progress towards care plan objectives and monitor member status or any changes in condition.

• Collaborate with healthcare providers on adjustments to care plans and refer members for additional care management evaluation when necessary.

• Collect, document, and maintain member information and care management activities to ensure regulatory compliance.

• May conduct on-site visits to assess member needs and collaborate with providers or resources.

• Carry out other assigned duties and adhere to all policies and standards.

⛳️ Requirements

• Bachelor’s degree along with 2–4 years of relevant experience.

• If possessing clinical licensure, must have graduated from an Accredited School of Nursing.

• Equivalent experience may be accepted if it reflects applicable knowledge, duties, scope, and skills appropriate for the position.

• Current clinical license in the state is preferred.

• Ability to work remotely from anywhere within the state of Missouri.

• Availability required Monday–Friday, from 8am to 5pm.

• Experience in foster care or pediatrics is strongly preferred.
