This is a fully remote position, open to applicants in Michigan.

📋 Description

• Develop, evaluate, and coordinate care management activities tailored to member needs.

• Create or assist in the formulation of personalized care plans/service plans.

• Provide education to members and their families/caregivers regarding services, benefit options, care alternatives, referrals, healthcare benefits, and provider guidelines.

• Assess member needs, identify barriers to care, evaluate available resources, and recommend/facilitate actionable plans.

• Identify necessary providers, specialists, and community resources for effective care.

• Offer psychosocial and resource support related to employment, education, housing, food security, participant direction, independent living, justice, and foster care requirements.

• Facilitate coordination between members, families, caregivers, and care provider teams to ensure prompt access to care and services.

• Track progress towards care plan objectives and monitor any changes in member status or condition.

• Collaborate with healthcare providers to update care plans and refer members for additional care management evaluation.

• Collect, document, and retain member information and care management activities to ensure regulatory compliance.

• Conduct on-site visits as necessary to assess member needs and work with providers/resources.

• Perform other assigned tasks while adhering to policies and standards.


⛳️ Requirements

• Bachelor’s degree.

• 2–4 years of relevant experience.

• Graduate from an Accredited School of Nursing if possessing clinical licensure.

• Equivalent experience gained through the achievement of relevant knowledge, duties, and skills reflective of this position's level may be acceptable.

• Current clinical license in the state preferred.

• Capability to work remotely from anywhere within the state of Michigan.

• Availability to work Monday through Friday, from 8:00 AM to 4:30 PM.

• Provide telephonic support for foster care children.
