Care Navigator

This is a fully remote position, open to applicants in Canada.

📋 Description

• Act as the primary contact for patients and their families.

• Assist older adults and caregivers throughout the intake process.

• Offer patient support, coordinate care, and provide emotional guidance.

• Pair patients with the appropriate therapist.

• Ensure patients feel understood and confident during their care journey.

• Oversee multiple patients at varying stages of the process.

• Take initiative in shaping essential processes.

• Collaborate closely with a thoughtful and cooperative remote team.

⛳️ Requirements

• Must be based in Canada.

• Must possess empathetic and clear communication skills.

• Should be highly organized and detail-oriented.

• Comfortable with learning Google Workspace, EHR platforms like Healthie, and CRMs such as HubSpot.

• Quick to learn new software applications.

• Interest in enhancing mental healthcare access for older adults.

• Experience in healthcare, customer service, or administrative support is a plus but not mandatory.

• Must be comfortable working hours in the Eastern Time Zone.

• Willingness to work one of the specified shifts: 9a–6p ET or 12p–9p ET.
