Care Navigator

This is a fully remote position, open to applicants in Canada.

📋 Description

• Act as the primary point of contact for patients or their families when they reach out to Sailor Health.

• Assist older adults and their caregivers through the intake process with compassion, clarity, and professionalism.

• Provide support to patients, coordinate care, and offer emotional assistance.

• Aid each new patient in navigating their entry into care.

• Connect patients with the appropriate therapist.

• Support patients and caregivers throughout the intake and care navigation experience.

• Ensure that older adults and their families feel acknowledged, supported, and cared for.

• Take the initiative and contribute to shaping essential processes within a small, dynamic team.

⛳️ Requirements

• Availability for full-time work.

• Strong empathetic and clear communication skills.

• Proficient in active listening and making individuals feel comfortable during challenging or emotional discussions.

• Highly organized with a strong attention to detail.

• Capable of managing multiple patients at various stages without overlooking tasks.

• Comfortable with learning Google Workspace, EHR systems like Healthie, and CRMs such as HubSpot.

• Quick to adapt to new software.

• Passionate about enhancing care for older adults.

• Previous experience in healthcare, customer service, or administrative support is beneficial but not mandatory.

• Must reside in Canada.

• Able to work hours aligned with Eastern Time Zone.

• Open to working one of the following shifts: Monday–Friday, Wednesday–Sunday, or Saturday–Wednesday, from 9 a.m. to 6 p.m. ET or 12 p.m. to 9 p.m. ET.
