Care Navigator

This is a fully remote position, open to applicants in Canada.

📋 Description

• Act as the initial contact for patients and their families.

• Assist older adults and caregivers throughout the intake procedure.

• Offer patient support, care coordination, and emotional assistance.

• Aid patients in finding the appropriate therapist.

• Support patients during their transition into care.

• Ensure that patients feel supported, understood, and reassured.

• Take the initiative and contribute to the development of essential processes.

• Work collaboratively with a dedicated, purpose-driven remote team.

⛳️ Requirements

• Candidates must be based in Canada.

• Availability for full-time work.

• Strong empathetic and clear communication skills.

• Proficient in active listening and able to assist individuals during challenging or emotional discussions.

• Highly organized with a keen attention to detail.

• Capable of managing multiple patients at various stages.

• Comfortable with learning Google Workspace, EHR platforms like Healthie, and CRMs such as HubSpot.

• Quick to adapt to new software applications.

• Genuine interest in mental health and improving healthcare access.

• Willing to work during Eastern Time Zone hours.

• Open to one of the specified shifts: 9a–6p ET or 12p–9p ET.
