Care Navigator

This is a fully remote position, open to applicants in Canada.

📋 Description

• Act as the initial point of contact for patients and their families reaching out to Sailor Health

• Assist older adults and caregivers through the intake process

• Offer patient support, care coordination, and emotional assistance

• Aid patients in navigating their care journey, including pairing them with the appropriate therapist

• Ensure patients feel cared for, understood, and confident throughout the intake process

• Take initiative and help develop essential processes within a small, agile team

⛳️ Requirements

• Strong empathetic and clear communication skills

• Proficient in active listening and capable of managing challenging or emotional discussions

• Highly organized with a keen attention to detail

• Ability to oversee multiple patients at various stages

• Comfortable using Google Workspace, EHR platforms like Healthie, and CRMs such as HubSpot

• Quick to learn new software applications

• Interest in mental health and accessibility to healthcare

• Candidates must reside in Canada

• Willingness to work during Eastern Time Zone hours

• Experience in healthcare, customer service, or administrative support is appreciated but not mandatory
