Care Navigator

This is a fully remote position, open to applicants in Canada.

📋 Description

• Act as the initial point of contact for patients and their families reaching out to Sailor Health.

• Assist older adults and caregivers throughout the intake procedure.

• Deliver warmth, clarity, and professionalism during the entire intake process.

• Coordinate patient care while offering emotional support.

• Aid patients in navigating their journey into care.

• Connect patients with the appropriate therapist.

• Ensure patients feel supported, confident, and understood throughout the entire experience.

⛳️ Requirements

• Applicants must reside in Canada.

• Must be an empathetic and clear communicator.

• Possess active listening skills and the ability to make others feel comfortable during challenging or emotional discussions.

• Highly organized with a keen attention to detail.

• Capable of managing multiple patients at various stages of their care.

• Must be comfortable learning Google Workspace, EHR systems like Healthie, and CRMs such as HubSpot.

• Ability to quickly adapt to new software.

• Interest in mental health and improving healthcare access.

• Experience in healthcare, customer service, or administrative roles is preferred but not mandatory.

• Willingness to work hours aligned with the Eastern Time Zone.

• Must be available for one of the specified shifts between 9 a.m. and 9 p.m. ET.
