Care Navigator

This is a fully remote position, open to applicants in United States.

📋 Description

• Handle inquiries from patients and durable power of attorney (DPOA) via telephone.

• Respond to messages from patients or caregivers submitted through the Curana Patient Portal.

• Assist patients in scheduling follow-up appointments with Curana Providers or specialists.

• Manage electronic health records (EHR) to ensure patient records are accurate and current.

• Coordinate documents that require review or signature by a provider.

• Facilitate provider orders and escalate findings as necessary.

• Maintain patient rosters for individuals enrolled in APCM and GUIDE programs.

• Support the scheduling of providers.

• Assist with prior authorizations as required.

• Obtain patient records and diagnostic test outcomes.

• Professionally and courteously manage incoming calls.

• Collaborate with the Curana Interdisciplinary Care Team to ensure effective communication within the health network.

• Perform additional duties as assigned.

⛳️ Requirements

• A high school diploma or equivalent is required.

• At least 1 year of experience in a medical office, Senior Living Community engagement, or similar fields.

• A minimum of 1 year of experience with Electronic Health Record (EHR) documentation or other practice management tools.

• A comprehensive understanding of medical terminology.

• Proficiency in interpreting medical records, lab results, and appointment notes.

• Basic knowledge of patient screening tools with the ability to identify changes over time.

• Capability to work in a distraction-free environment.

• Strong organizational and time management skills with the ability to prioritize tasks effectively.

• Skilled in managing multiple tasks at the same time.

• Proficient computer skills with the ability to adapt to various technology platforms.

• Prior experience in virtual triage is preferred.

• Bilingual or multilingual communication skills are preferred.

• A reliable high-speed internet connection is required.
