Care Navigator

This is a fully remote position, open to applicants in United States.

📋 Description

• Handle inquiries from patients and durable power of attorney (DPOA) via phone.

• Address messages from patients or caregivers received through the Curana Patient Portal.

• Assist patients in scheduling follow-up appointments with Curana Providers or specialists.

• Oversee electronic health records (EHR) and ensure patient records are accurate and current.

• Organize documents for provider review or signature.

• Facilitate provider orders and escalate findings as necessary.

• Maintain patient rosters for individuals enrolled in APCM and GUIDE programs.

• Support provider scheduling efforts.

• Assist with prior authorization processes.

• Acquire patient records and diagnostic test results.

• Manage incoming calls with professionalism and courtesy.

• Work collaboratively with the Curana Interdisciplinary Care Team to promote effective communication.

• Carry out other assigned responsibilities.

⛳️ Requirements

• Possession of a high school diploma or equivalent qualification.

• At least 1 year of experience in a medical office, Senior Living Community engagement, or related fields.

• Minimum of 1 year of experience with Electronic Health Record (EHR) documentation or similar practice management tools.

• Comprehensive knowledge of medical terminology.

• Capability to interpret medical records, lab results, and appointment notes.

• Basic understanding of patient screening tools and recognizing changes over time.

• Ability to maintain focus in a distraction-free environment.

• Exceptional organizational and time management skills, with the aptitude to prioritize tasks effectively.

• Proficient in managing multiple tasks concurrently.

• Competent computer skills and adaptability to various technology platforms.

• Must have a reliable high-speed internet connection.
