Care Navigator

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Deliver concierge-level assistance to members throughout their healthcare journey

• Aid members in locating curated lists of high-quality, cost-effective in-network specialists

• Clarify medical plans, covered benefits, financial responsibilities, employer point solutions, and cost-containment initiatives

• Support appointment scheduling and provide continuous assistance

• Utilize web-based tools to research providers and maximize member benefits

• Oversee daily tasks in internal systems within designated turnaround times

• Address inquiries from both members and providers

• Verify appointment attendance and manage scheduling

• Record service impacts and savings for precise value reporting

• Collaborate with internal and external partners to enhance member care coordination

• Document updates and ongoing impacts in the relevant systems

• Create and uphold written operational procedures

• Train team members across various functions

• Work with Medical Management Leadership to strategize on trends identified in member cases

• Perform additional duties as assigned

⛳️ Requirements

• Bachelor's degree or equivalent professional experience is required

• A minimum of 2 years of experience in member advocacy or care coordination

• Familiarity with employee medical benefit plans

• Preferred experience with group health insurance and self-funded health plans

• Excellent written and verbal communication skills

• Intermediate-level proficiency with Microsoft Office suite

• Reliable internet connection via cable broadband or fiber optic service

• Minimum internet speeds of 100Mbps for download and 25Mbps for upload
