Care Model Management Advisor

This is a fully remote position, open to applicants in Virginia.

📋 Description

• Take charge of the daily execution of a specified care model workstream.

• Accomplish tasks and milestones according to the established plan.

• Create and uphold task-level work plans.

• Monitor actions, owners, dependencies, and deadlines.

• Collaborate with functional contributors to ensure timely delivery of workstream outputs.

• Recognize and manage risks and issues at the workstream level.

• Suggest options for mitigation and assess timing impacts.

• Report any risks, issues, or delays that may affect the overall care model critical path.

• Provide precise and timely status updates for integrated workstream reporting.

• Maintain documentation for the workstream, including action logs and supporting materials.

• Ensure operational excellence, meticulous attention to detail, and accountability for assigned outcomes.

• Act as an operational partner to the Workstream PM and Senior Care Model leadership.

⛳️ Requirements

• 3–5 years of experience in healthcare operations, care model implementation, clinical programs, or similar operational roles that support care delivery or value-based initiatives.

• Practical experience in managing workstreams or projects, including task-level planning, execution, monitoring, and follow-up within a defined scope.

• Proven ability to coordinate cross-functional contributors, manage dependencies, and drive deliverables to completion without direct authority.

• Demonstrated expertise in identifying and managing risks and issues, along with proposing mitigation strategies and appropriately escalating impacts.

• Experience in generating detailed status reports and maintaining work artifacts such as action logs, timelines, and documentation.

• Familiarity with healthcare delivery systems and regulatory requirements.

• Bachelor's degree in healthcare administration, nursing, public health, or a related field preferred, or equivalent experience.

• Must be a resident of the United States.
