Care Manager, Transition of Care (RN)

This is a fully remote position, open to applicants in California.

📋 Description

• Evaluate, plan, and organize medical and support services for members after discharge.

• Perform medication assessments and reconcile medications before and after admission.

• Create care/service plans to assist members with their transition and discharge requirements.

• Analyze member health conditions, resource needs, services, and treatment strategies; deliver suitable interventions.

• Assist in the transition to active care management based on the specific needs of the member.

• Provide or arrange educational resources and materials for members, caregivers, and healthcare providers.

• Coordinate services among primary care physicians, specialists, medical practitioners, and non-medical resources.

• Conduct outreach through phone calls, digital means, home visits, or other locations.

• Gather, document, and sustain member information and care management activities.

• Offer leadership insights on potential improvements for care quality and cost efficiency.

• Adhere to policies and standards while performing additional assigned tasks.

⛳️ Requirements

• A valid California RN license is mandatory.

• Must hold a license in CA.

• A degree from an accredited nursing school along with 2–4 years of relevant experience, or a Master’s degree in Behavioral Health or Social Work is required.

• Licenses such as LISW, LCSW, LMSW, LMFT, LMHC, LPC, or RN are necessary.

• Residency in California is preferred.

• Strong preference for experience in discharge planning, care coordination, and postpartum care.

• Capability to conduct post-discharge assessments and medication reconciliation.

• Ability to work collaboratively with discharge planners, providers, specialists, and interdisciplinary teams.

• Familiarity with current state, federal, and third-party payer regulations is essential.
