Care Manager, RN

This is a fully remote position, open to applicants in New Jersey.

📋 Description

• Design, assess, and implement intricate care management initiatives for members with predominantly physical needs.

• Assess member requirements, barriers to access, and social determinants impacting health.

• Create ongoing care plans while collaborating with healthcare providers, specialists, and community resources.

• Identify issues and obstacles to care, providing necessary care management interventions.

• Coordinate person-centered care among members, their caregivers, and provider teams.

• Follow up and track member status, changes in condition, and progress towards care objectives.

• Offer local resource support for services related to employment, housing, participant direction, independent living, justice, and foster care.

• Facilitate care management and ensure timely access to essential services.

• Conduct outreach through telephonic, digital, home visits, and other locations.

• Collect, document, and preserve member information and care management activities to ensure regulatory compliance.

• Educate members and their families/caregivers on disease processes, care gaps, provider instructions, care options, referrals, and healthcare benefits.

• Provide leadership insights on opportunities to enhance care quality and cost-effectiveness.

• Execute other assigned responsibilities while adhering to established policies and standards.

⛳️ Requirements

• Degree from an accredited nursing school or a Bachelor’s degree in Nursing.

• 2–4 years of relevant experience.

• Active RN license in New Jersey is mandatory.

• RN - Registered Nurse - State Licensure and/or Compact State Licensure is required.

• Candidates must reside in Bergen or Essex County.

• Availability to work Monday through Friday, 8:00 AM to 5:00 PM EST, with flexibility based on member and business needs.

• Willingness to travel locally up to 50% for in-home member visits.
