Care Manager, RN

This is a fully remote position, open to applicants in Mississippi.

📋 Description

• Design, evaluate, and implement comprehensive care management strategies primarily aimed at members with physical needs.

• Create individualized care plans and offer educational resources for members and their families.

• Assess member requirements, obstacles to receiving appropriate care, and social determinants affecting health.

• Formulate continuous care plans while collaborating with healthcare providers, specialists, and community organizations.

• Recognize care-related issues and obstacles, providing suitable interventions.

• Facilitate communication among members, families/caregivers, and healthcare provider teams.

• Monitor and follow up on member status, any changes in condition, and progress towards goals outlined in care plans.

• Offer resource support related to employment, housing, participant direction, independent living, justice, and foster care services.

• Manage care coordination and ensure timely access to necessary services or care.

• Conduct outreach via telephone, digital platforms, home visits, and other locations.

• Gather, record, and maintain member data and care management activities to ensure compliance with regulations.

• Educate members and their families/caregivers about disease processes, care gaps, provider directives, care alternatives, referrals, and healthcare benefits.

• Provide constructive feedback to leadership on enhancing care quality and cost-effectiveness.

• Adhere to all policies and standards, and undertake other assigned responsibilities.

⛳️ Requirements

• An active Registered Nurse (RN) license is required in the state of Mississippi.

• Candidates must reside in Mississippi.

• Possession of a degree from an Accredited School of Nursing or a Bachelor's degree in Nursing is necessary.

• 2–4 years of relevant experience is preferred.

• RN - Registered Nurse - State Licensure and/or Compact State Licensure is mandatory.

• Experience in assessment and case management is desirable.

• Must live and hold a license in Mississippi.
