Care Manager, RN

This is a fully remote position, open to applicants in Arizona.

📋 Description

• Design, evaluate, and implement intricate care management activities for members primarily with physical health needs.

• Assess member requirements, barriers to accessing care, and social determinants of health, concentrating on priorities identified by members.

• Create ongoing care and service plans.

• Work collaboratively with providers, specialists, and community resources to meet unmet needs.

• Recognize problems and obstacles to care, providing suitable care management interventions.

• Coordinate efforts among members, families/caregivers, and care provider teams to ensure a person-centered approach to care.

• Monitor and follow up on member status, changes in health condition, and progress toward care plan objectives.

• Update and revise care plans as needed in collaboration with members, caregivers, and providers.

• Offer resource support for local services, including employment, housing, independent living, justice, and foster care.

• Facilitate care management and work with providers or specialists to ensure prompt access to care.

• Conduct outreach via telephone, digital platforms, home visits, and/or other sites.

• Gather, document, and preserve member information and care management activities to ensure regulatory compliance.

• Educate members and their families/caregivers about disease processes, care gaps, provider instructions, care alternatives, referrals, and healthcare benefits.

• Provide constructive feedback on opportunities to enhance care quality and cost-effective service delivery.

• Carry out other responsibilities assigned by the team leader while adhering to policies and standards.

⛳️ Requirements

• Degree from an accredited nursing school or a Bachelor's degree in Nursing.

• 2–4 years of relevant experience.

• RN (Registered Nurse) state licensure and/or compact state licensure is required.

• Must be a resident of Arizona.
