Care Manager, Registered Nurse – Field

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Employ critical thinking and judgment to engage in and enhance the case management process.

• Promote favorable healthcare outcomes for members.

• Assist in scheduling appointments.

• Identify and help members access their benefits.

• Educate members utilizing care management tools and resources.

• Work collaboratively with members, providers, and community organizations.

• Address the healthcare and social determinant needs of members.

• Travel up to 75% from the home location across Metro East/Southern Illinois and nearby regions.

⛳️ Requirements

• Must be a resident of Illinois.

• Must have reliable transportation and be willing to travel up to 75% of the time from the home location in and around Metro East/Southern Illinois.

• A minimum of 3-5 years of clinical practical experience is required.

• Confidence in working independently from home and using tools to connect and collaborate with teams virtually.

• Strong analytical and problem-solving abilities.

• Excellent communication, organizational, and interpersonal skills.

• Ability to work autonomously.

• Proficient computer skills, including the ability to navigate multiple systems and keyboard efficiently.

• Proficiency in MS Word, Excel, Outlook, PowerPoint, and proprietary applications.

• An Associate's degree is mandatory.

• Must hold an active and unencumbered Registered Nurse license in Illinois.

• Direct/hardwired internet connection to a modem/router within 7 feet of the computer is necessary.

• Minimum internet speed of 25 Mbps download and 3 Mbps upload is required.

• A quiet, secure, and private designated home virtual work location that complies with CVS Health and HIPAA guidelines is essential.

• Must work within the state and city of current residence.

• Workspace furniture must be provided by the applicant.

• Experience in setting up and using multiple monitors while navigating various applications is preferred.

• Familiarity with Microsoft Office 365 applications or similar Google Workspace tools is necessary.

• Ability to access secure systems such as VPN and EHR portals is required.

• Must manage strong passwords and recognize suspicious emails or links.

• Capability to resolve common technical issues independently is essential.

• Willingness to learn new skills as the workplace evolves is crucial.
