Care Manager – Occupational Therapy

This is a fully remote position, open to applicants in Alabama, +19 more states.

📋 Description

• Oversee comprehensive care management and ensure high-quality care for participants utilizing discipline-specific expertise.

• Evaluate assessments and documentation from field staff for quality assurance, address any deficiencies, and oversee interventions.

• Engage in care-planning meetings and contribute as an integral member of the interdisciplinary team (IDT).

• Develop and coordinate holistic care plans while maintaining regular communication with discipline-specific field personnel.

• Organize, coordinate, and authorize services that cater to member needs, which include activities of daily living (ADLs/IADLs), housing, meals, transportation, skilled services, and long-term care.

• Collaborate with physicians, IDT members, healthcare facilities, discharge planners, and home care coordinators across various care environments.

• Implement, monitor, assess, and adjust care management interventions and plans based on member outcomes and evolving health requirements.

• Record services in accordance with CLHC standards and adhere to federal and state regulations.

• Facilitate coaching, competency assessments, and training sessions for field staff.

• Propose enhancements to services, programs, policies, and procedures.

• Adhere to disciplinary policies and consult Human Resources when necessary.

• Operate within the scope of practice and complete all required competencies prior to working independently.

• Perform additional duties as assigned.

⛳️ Requirements

• Graduation from an Occupational Therapy program recognized by the New York State Education Department (NYSED).

• At least 2 years of administrative experience in a managerial role within a certified home health agency (CHHA), rehabilitation, long-term home health care (LHCSA), acute care, medical-surgical, critical care, nursing home settings, or diagnostic & treatment clinics is preferred.

• Customer service experience is mandatory.

• Experience managing long-term care insurance is advantageous.

• A minimum of one (1) year of experience working with a frail or elderly population is required, or appropriate PACE-organization training upon hiring if less than one year and all other criteria are met.

• Supervisory experience is desirable.

• Must be legally authorized to practice in the state where the healthcare professional will perform their duties.

• Must be medically cleared for communicable diseases and have all immunizations current before engaging in direct participant interaction.

• Capability to fulfill specified physical requirements, including standing, sitting, lifting/pushing/pulling up to 50 pounds, bending/squatting, walking/climbing, traveling between sites, fine motor skills, visual acuity, auditory/speaking skills, and cognitive decision-making abilities.
