This is a fully remote position, open to applicants in Massachusetts.
• Engage in member outreach through phone calls and in-person meetings to facilitate program enrollment.
• Conduct comprehensive assessments and evaluate members holistically to recognize their needs, health objectives, and obstacles to wellness.
• Create, implement, and modify individualized care plans and interventions for care management.
• Ensure documentation is completed in accordance with departmental policies and regulatory standards.
• Deliver focused health education and proactive strategies for condition management.
• Communicate effectively with providers and vendors involved in the care of members.
• Coordinate safe transitions to subsequent levels of care, including post-discharge instructions, services, and follow-up.
• Employ strategies aimed at preventing readmissions.
• Work collaboratively with interdisciplinary care teams to enhance member outcomes.
• Attend interdisciplinary rounds and present high-risk members.
• Foster professional development through self-directed learning.
• Must be a Registered Nurse with a current unrestricted license in the state of residence.
• May need to secure additional state licensure in the states where Point32Health operates.
• Must understand and adhere to the provisions of state-specific Nurse Practice Act(s) in the areas where Point32Health operates.
• National certification in Case Management is preferred.
• Bachelor’s degree or equivalent relevant experience is required.
• A minimum of 5 years of relevant clinical experience is necessary.
• Experience in home care or case management is preferred.
• Proficiency in a second language is a plus.
• Experience in specialty areas is advantageous.
• Must possess skills and proficiency in Microsoft applications, technical concepts and principles, and various computer software applications.
• Demonstrated skills in assessment, planning, and managing member care are essential.
• Advanced communication and interpersonal skills are required.
• Ability to address multiple complex issues effectively.
• Capable of organizing and prioritizing work as well as member needs.
• Strong clinical and critical thinking abilities are essential.
• Must maintain confidentiality of data and adhere to corporate compliance policies.
• Ability to make face-to-face visits as required.
• A valid driver’s license and a vehicle in good working order are necessary.
