This is a fully remote position, open to applicants in Missouri.
• Design, evaluate, and implement intricate care management strategies for members primarily facing mental and behavioral health challenges.
• Develop tailored care plans and offer educational support to members and their families concerning mental health and substance use disorders.
• Assess member requirements through phone consultations or in-home visits, recommending or facilitating appropriate care and service plans.
• Perform outreach via phone, digital platforms, home visits, and other locations to evaluate needs and collaborate with available resources.
• Create ongoing care plans for high-acuity members and identify suitable providers, specialists, and community resources.
• Coordinate efforts among members, families or caregivers, community resources, and care provider teams.
• Monitor care plans, member conditions, treatment-related side effects, complications, and clinical symptoms.
• Facilitate care coordination with providers and specialists to ensure prompt access to necessary care.
• Gather, document, and maintain member information and care management activities to comply with regulatory requirements.
• Educate members and their families about procedures, provider guidelines, treatment options, referrals, and healthcare benefits.
• Provide feedback on leadership opportunities to enhance care delivery and quality in a cost-effective manner.
• Execute additional tasks as assigned while adhering to policies and standards.
• A Master's degree in Behavioral Health or Social Work, or a degree from an accredited nursing school.
• 2–4 years of pertinent experience.
• Licensed Master's Behavioral Health Professional, such as LCSW, LMSW, LMFT, LMHC, or LPC, or RN in accordance with state contract requirements.
• Behavioral health experience is mandatory for RN candidates.
• Strong capability to develop and manage complex behavioral health care plans.
• Proficient in evaluating member needs through phone and in-home assessments.
• Skilled in coordinating efforts with providers, specialists, families, caregivers, and community resources.
• Ability to document care management activities in accordance with state, federal, and third-party payer regulations.
• Availability from Monday to Friday, 8:00 a.m. to 5:00 p.m.
• LCSW, LMSW, LMFT, LMHC, or LPC license is highly preferred.
