Care Facilitator – Medical Assistant

This is a fully remote position, open to applicants in Kentucky, +1 more state.

📋 Description

• Address incoming inquiries from patients and their families, escalating urgent triage calls to the clinical team as necessary.

• Deliver prompt, precise, and supportive information to patients.

• Initiate proactive outreach to patients for ongoing care management.

• Organize and schedule appointments as well as referrals.

• Enhance communication and collaboration among healthcare providers, patients, and their families.

• Act as a liaison between services and external providers.

• Track patients' progress and responses to treatment, identifying any changes or complications.

• Relay escalations or emerging health risks to clinical leadership.

• Keep accurate communication logs and adhere to documentation standards.

• Engage in quality improvement and performance enhancement initiatives.

• Recognize and implement strategies to improve care quality.

• Comply with HIPAA regulations and protect patient confidentiality.

• Ensure adherence to clinical guidelines and standards of care.

⛳️ Requirements

• Some college coursework or an associate degree in clinical sciences, Nursing, or a related discipline.

• A minimum of two (2) years of experience in gathering referral information within the healthcare sector.

• Medical Assistant certification is preferred but not mandatory.

• Familiarity with electronic medical record software.

• Proficient in the Microsoft Office suite.

• Capability to multitask, prioritize, and handle multiple projects concurrently.

• Exceptional written and verbal communication abilities.

• Ability to work autonomously as well as collaboratively in a team environment.
