Care Facilitator – Medical Assistant

This is a fully remote position, open to applicants in Kentucky, +1 more state.

📋 Description

• Address incoming inquiries from patients and their families, escalating urgent triage calls to the clinical team as needed.

• Deliver prompt, accurate, and helpful information to patients.

• Proactively engage with patients for ongoing care management.

• Organize and schedule appointments and referrals efficiently.

• Foster communication and collaboration among healthcare providers, patients, and their families.

• Act as a liaison between services and external healthcare providers.

• Track patients’ progress and responses to treatment protocols.

• Recognize changes or complications and relay escalations or new health risks to clinical leadership.

• Keep precise and current communication logs.

• Comply with documentation standards and regulatory requirements.

• Engage in quality improvement initiatives and performance enhancement projects.

• Identify and implement enhancements in care coordination, patient outcomes, and overall patient satisfaction.

• Follow HIPAA regulations and ensure patient confidentiality is maintained.

• Guarantee adherence to clinical guidelines and standards of care.

⛳️ Requirements

• Some college education or an associate degree in clinical sciences, nursing, or a related discipline.

• At least two (2) years of experience in gathering referral information within the healthcare sector.

• Medical Assistant certification is preferred but not mandatory.

• Familiarity with electronic medical record software is essential.

• Proficient in the Microsoft Office suite.

• Strong ability to multitask, prioritize, and manage several projects at once.

• Exceptional written and verbal communication skills.

• Capacity to work independently or collaboratively within a team environment.
