This is a fully remote position, open to applicants in Minnesota.

📋 Description

• Independently oversee a designated caseload of members.

• Develop personalized care plans.

• Coordinate authorized medical and social services.

• Provide education to members regarding their Medicare and Medicaid benefits.

• Act as the primary point of contact for member inquiries and concerns.

• Collaborate with physicians, providers, and county financial organizations to formulate care plans.

• Offer updates on the progress of members.

• Conduct assessments via telephone and in-person.

• Self-manage and schedule work according to member availability and regulatory standards.

• Execute additional responsibilities as assigned.


⛳️ Requirements

• Must reside in Minnesota and support members within the state.

• For RN: RN diploma or degree in Nursing (either Associate's or Bachelor's) with at least 2 years of experience in community-based services, OR a Bachelor's degree in Nursing with a Public Health Certification and a minimum of 1 year of experience in community-based services.

• For RN: 3-5+ years of experience beyond the degree.

• For SW: Bachelor's degree in Social Work.

• For SW: 3-5+ years of experience beyond the degree, including 1-2+ years in community-based services.

• Possess an active, unrestricted RN License or SW License in Minnesota.

• Preferred: MnCHOICES Certified Assessor Training (MNCAT) or the ability to obtain it within 21 days of employment.

• Experience in home and community-based services/case management is preferred.

• Experience working with individuals with disabilities and seniors is preferred.

• Comfortable engaging with diverse, low-income populations is preferred.

• Strong knowledge of Medicare and Medical Assistance programs is preferred.

• Excellent interpersonal skills with the ability to convey medical information in simple terms for non-medically trained members.

• Ability to work independently while also collaborating as a team player, shifting priorities, and reprioritizing tasks as needed.

• Strong clinical and documentation abilities.

• Innovative problem-solving skills.

• Exceptional organizational capabilities.

• Willingness to travel within Medica's Service Area.

• Must have reliable transportation.

• A designated private area in the home for a home office setup.

• General comfort with technology and proficiency in Microsoft Office Suite.

• Legally authorized to work in the United States at the time of application; Medica does not provide work visa sponsorship.
