This is a fully remote position, open to applicants in Minnesota.
• Manage a designated caseload of members independently.
• Develop personalized care plans for each member.
• Coordinate the delivery of sanctioned medical and social services.
• Educate members regarding their Medicare and Medicaid benefits.
• Act as a reliable primary contact to resolve and alleviate member inquiries and concerns.
• Collaborate with physicians, providers, and county financial organizations concerning care plans.
• Provide updates on the progress of members.
• Conduct member assessments via telephone and in-person visits.
• Schedule and manage work autonomously based on member availability and regulatory stipulations.
• Execute additional duties as assigned.
• RN Diploma or Degree in Nursing (Associate or Bachelor’s) with at least 2 years of experience in community-based services, OR a Bachelor’s degree in Nursing with a Public Health Certification and a minimum of 1 year of experience in community-based services.
• Over 5 years of experience beyond the degree.
• Active, unrestricted RN License in Minnesota.
• MnCHOICES Certified Assessor Training (MNCAT) preferred, or the ability to obtain it within 21 days of employment.
• Public Health Certification preferred but not mandatory.
• Experience in home and community-based services/case management.
• Familiarity with working with individuals with disabilities and seniors.
• Comfortable engaging with diverse, low-income populations.
• Strong understanding of Medicare and Medical Assistance programs.
• Excellent interpersonal abilities and the capacity to simplify complex medical terminology for non-medically trained members.
• Capability to work independently while also being a collaborative team player, adapt quickly to changes, and reprioritize tasks as needed.
• Strong clinical and documentation skills.
• Exceptional innovative problem-solving skills.
• Outstanding organizational capabilities.
• Willingness to travel within Medica's Service Area.
• Reliable transportation is required.
• A designated private area of the home for a home office setup.
• General comfort with technology and proficiency in Microsoft Office Suite.
• Must reside in Minnesota.
• Must be legally authorized to work in the United States; Medica does not sponsor work visas.
