Care Coordinator II, III – RN, Licensed SW

This is a fully remote position, open to applicants in Minnesota.

📋 Description

• Independently manage a designated caseload of members.

• Develop personalized care plans.

• Coordinate authorized medical and social services.

• Inform members about their Medicare and Medicaid benefits.

• Act as the primary contact to resolve member inquiries and concerns.

• Collaborate with physicians, providers, and county financial entities regarding care plans.

• Provide updates regarding member progress.

• Conduct both telephonic and in-home assessments of members.

• Schedule and organize work according to member availability and regulatory guidelines.

• Travel within the community to meet members in their residences.

• Carry out additional responsibilities as assigned.

⛳️ Requirements

• Must be a Minnesota resident and support membership within the state.

• RN: A diploma or degree in Nursing (Associate's or Bachelor's) with at least 2 years of experience in community-based services, OR a Bachelor's degree in Nursing with a Public Health Certification and a minimum of 1 year of experience in community-based services.

• RN: 3–5+ years of experience beyond the degree.

• SW: Bachelor's degree in Social Work.

• SW: 3–5+ years of experience beyond the degree, including 1–2+ years in community-based services.

• Active, unrestricted RN License or SW License in Minnesota.

• MnCHOICES Certified Assessor Training (MNCAT) preferred or the ability to obtain it within 21 days of employment.

• Public Health Certification preferred, but not mandatory.

• Experience in home and community-based services/case management is preferred.

• Familiarity with working with individuals with disabilities and seniors is preferred.

• Comfortable engaging with diverse, low-income populations.

• Strong understanding of Medicare and Medical Assistance programs.

• Excellent interpersonal skills with the ability to simplify medical terminology for members without medical training.

• Capable of working independently while also being a collaborative team player, with the ability to shift priorities and reassign tasks as needed.

• Strong clinical and documentation capabilities.

• Willing to travel within Medica's Service Area.

• A designated private area in the home for office use.

• General proficiency with technology and familiarity with Microsoft Office Suite.
