Care Coordinator II – Foster Care

This is a fully remote position, open to applicants in Missouri.

📋 Description

• Assist in care management activities and collaborate with assigned member teams

• Ensure that services are rendered by healthcare providers and associated partners

• Conduct member outreach through phone calls or home visits

• Record care and service activities as well as plans

• Discuss care plans, service plans, next steps, resources, questions, and concerns with members

• Coordinate care activities with healthcare and community providers, partners, members, and caregivers

• Address inquiries, requests, and concerns from members and providers

• Communicate with care managers, practitioners, and others to facilitate member services and ensure continuity of care

• Assist in service assessments and screenings

• Maintain and distribute member records as required

• Identify member needs and refer them to Care Managers, community-based organizations, and Disease Managers

• Provide education on available benefits and resources

• Perform additional duties as assigned while adhering to policies and standards

⛳️ Requirements

• High School diploma or GED is required

• 1–2 years of relevant experience is necessary

• Must be able to work remotely from anywhere within the state of Missouri

• Availability to work Monday–Friday, from 8am to 5pm

• Capability to provide telephonic support to children in Foster Care

• Ability to conduct member outreach via phone or home visits

• Proficiency in coordinating care with healthcare and community providers, partners, members, and caregivers

• Competence in documenting and maintaining member records in compliance with state and regulatory requirements

• Adherence to contractual obligations, regulatory guidelines, and standards is essential
