Care Coordinator II – Foster Care

This is a fully remote position, open to applicants in Missouri.

📋 Description

• Assist with care management activities and designated member teams

• Conduct member outreach via phone calls or in-person home visits

• Record care and service plans along with related activities

• Collaborate with healthcare providers, community partners, members, and caregivers to coordinate care

• Address inquiries, requests, and concerns from members and providers

• Liaise with care managers, practitioners, and other stakeholders to ensure service delivery and continuity of care

• Aid in service assessments and screenings

• Keep member records in compliance with state and regulatory standards

• Recognize needs and refer to Care Manager, community-based organizations, and Disease Manager

• Educate members about available benefits and resources

• Execute other assigned duties and adhere to established policies and standards

⛳️ Requirements

• High School diploma or GED equivalent

• 1–2 years of relevant experience

• Capability to work remotely from home anywhere in the state of Missouri

• Availability from Monday to Friday, 8am to 5pm

• Proficiency in performing member outreach through phone calls and home visits

• Understanding of care plans/service plans and member care requirements

• Skill in identifying needs and making referrals to Care Manager, community-based organizations, and Disease Manager

• Adherence to state, regulatory, contractual, and organizational guidelines
