Care Coordinator II, Foster Care

This is a fully remote position, open to applicants in Missouri.

📋 Description

• Assist in care management tasks and collaborate with assigned member teams

• Ensure that services are provided by healthcare professionals and partners

• Engage with members via telephone outreach and home visits

• Record care and service activity plans meticulously

• Discuss care strategies, subsequent steps, resources, inquiries, and concerns with members

• Organize care activities alongside healthcare and community providers, partners, members, and caregivers

• Address inquiries and concerns from both members and providers

• Communicate effectively with care managers, practitioners, and other stakeholders to enhance member services and ensure continuity of care

• Aid in service assessments and screenings

• Document and manage member records, ensuring distribution to providers as required

• Identify member needs and refer them to Care Managers, community organizations, and Disease Managers

• Educate members on available benefits and resources

• Execute other assigned tasks while adhering to policies and standards

⛳️ Requirements

• High School diploma or GED is required

• 1–2 years of relevant experience preferred

• Capability to work remotely from home anywhere within Missouri

• Availability from Monday to Friday, 8 am to 5 pm

• Competence in providing telephonic support to Foster Care children

• Ability to conduct member outreach through phone calls or home visits

• Proficiency in coordinating care with healthcare and community providers, partners, members, and caregivers

• Skill in documenting and maintaining member records in compliance with state and regulatory standards

• Understanding of contractual obligations, regulatory guidelines, and standards
