This is a fully remote position, open to applicants in Wisconsin.
• Assist in care management activities along with assigned member teams
• Ensure that services are provided by healthcare providers and partners
• Conduct member outreach via telephone and home visits
• Record care/service plans and associated activities
• Engage in discussions about care plans, next steps, resources, questions, concerns, and education with members
• Coordinate care activities with healthcare and community providers, partners, members, and caregivers
• Address member and provider inquiries, requests, and concerns
• Communicate with care managers, practitioners, and others to facilitate member services and ensure continuity of care
• Assist with service assessments and screenings, documenting member care needs
• Maintain and distribute member records as necessary
• Recognize needs and make referrals to Care Managers, community-based organizations, and Disease Managers
• Provide education regarding available benefits and resources
• Carry out other assigned duties and adhere to policies and standards
• High School diploma or GED
• 1–2 years of relevant experience
• Capability to conduct member outreach through phone calls and home visits
• Ability to coordinate care with healthcare and community providers, partners, members, and caregivers
• Proficiency in documenting and maintaining member records following state and regulatory standards
• Compliance with contractual requirements, regulatory guidelines, and standards
• Previous experience in assessments and/or call center work is preferred
• Availability to work Monday–Friday, from 8 am to 5 pm Central
